Insurance Plan
We accept a wide variety of insurances!
Most insurance plans include coverage for your physical therapy visits. If you are unsure about the details of your plan, be sure to contact your insurance company before your physical therapy visit to verify your benefits including number of visits, deductible, out-of-pocket maximum, or if a referral is required.
We offer a complimentary outline of insurance benefits for all of our patients, however, it is the patient's responsibility to understand your insurance coverage prior to your visit. Feel free to call us with any questions!
Cash-Based Plan
We also offer cash payments to cover your physical therapy visits. This might be the option for you if you have an insurance plan with a high copay, don't have insurance, or have a high deductible insurance plan in which you have an HSA.
You do not need a referral if you choose this plan.

What to Expect

If applicable, you'll be expected to to pay your co-pay, co-insurance, or amount toward your deductible at the time of service.

Please arrive 15 minutes early for your first appointment if you choose to complete your intake paperwork at your visit.

Please bring your insurance card and a proof of identification to your first visit

Wear comfortable clothes that are easy for you to move around in.

Initial evaluations and follow-up visits are 45-60 minutes long.
