New Patient Paperwork
Save time by filling out your intake form either online or by printing your paperwork prior to your appointment. Completing your intake paperwork ahead of time allows your physical therapist more time to review your information.
If you are a current patient and you can't find an appointment time....
Physical Therapy Visit Coverage
We accept a wide variety of insurances!
Most insurance plans include coverage for your physical therapy visits. If you are unsure about the details of your plan, be sure to contact your insurance company before your physical therapy visit to verify your benefits including number of visits, deductible, out-of-pocket maximum, or if a referral is required.
It is the patient's responsibility to understand your insurance coverage prior to your visit.
Feel free to call us with any questions!
