Physical Therapy Visit Coverage
Insurance Plan
We accept a wide variety of insurances!
Most insurance plans include coverage for your physical therapy visits. If you are unsure about the details of your plan, be sure to contact your insurance company before your physical therapy visit to verify your benefits including number of visits, deductible, out-of-pocket maximum, or if a referral is required.

It is the patient's responsibility to understand your insurance coverage prior to your visit. All patients are required to have a card on file at our clinic. Patients who have a copay or select the "self-pay" option are required to pay that fee at time of service. Feel free to call us with any questions!
What to Expect
If applicable, you'll be expected to pay your co-pay, co-insurance, or amount toward your deductible at the time of service.

You will receive a link to complete your initial visit paperwork online. If you choose to complete your paperwork at your visit, please arrive 15-minutes early for your first appointment.

Please bring your insurance card and a proof of identification to your first visit.
​
Wear comfortable clothes that are easy for you to move around in.
​
Initial evaluations and follow-up visits are typically 55-minutes long.

To reschedule or cancel, please provide a 24-hour notice or you will be charged our late cancellation / no-show fee.
